Minimum record:
- Inquiry ID and received time
- Stated need and chosen reply channel
- Consent relevant to the follow-up
- Assigned human owner
- Current state and last transition reason
- Next action and due time

Operational checks:
- Repeated requests do not duplicate the inquiry or acknowledgement.
- A delivery failure is visible to a person.
- Every open inquiry has an owner or appears in an unassigned queue.
- Withdrawal stops automated follow-up.
- Close only after recording the outcome.

Review: time to first human response, unassigned inquiries, unresolved failures.
